For your convenience, we have provided a way for you to complete your new patient forms in the privacy of your home. Simply download and print them, fill them out and bring them with you to your appointment.

If You Have Dental Insurance
We will do everything possible to help you maximize your dental benefits. We currently accept all private care insurances, which means that we work with literally thousands of companies. Although we can maintain computerized histories of payment by a given company, they do change. Therefore, we estimate your portion based on the most current information available. Our office does require payment for your portion at the time of service.
Affordable Treatment
We believe that you shouldn't put off necessary - or desired - dental care because of cost. For this reason, we offer options that can make treatment affordable.
One option is the convenience and affordability of third party financing with Care Credit which utilizes many no interest payment options.
